Notes:

1. First time skaters regardless of their division will pay the HNL insurance fee of $51.00 plus a survey fee of $25.00 to be refunded upon completion of the survey at season’s end and return to the Association. In the event that the survey is not     completed the player will be charged the full amount of registration for the division they participated in.

2 Families with more than one child will full registration for the first child plus HNL insurance fee and one half the registration fee for each subsequent child plus   the full HNL insurance fee. For purposes of this rule the oldest child is deemed to be the first child.

3. Females eligible for U12 or U20 must register in the respective division first, if that female player wishes to participate in Atom, Peewee, Bantam or Midget House League they must pay an additional fee of $100.00.

4. Any player wishing to participate in EVMHA Rep Hockey must pay an additional fee of $150.00.
